A tooth doesn't literally "die" in the same way a person or organ does. When people say a tooth nerve has died, they usually mean that the pulp—the living tissue containing nerves and blood vessels inside the tooth—has become irreversibly damaged or has lost its blood supply.
A dead or non-vital tooth may stop responding to hot and cold, but that doesn't necessarily mean the problem is gone. In fact, bacteria can eventually enter the damaged pulp space and cause an infection around the root.
Without appropriate dental treatment, a dead tooth can become painful, develop an abscess, or damage the surrounding bone.
If you suspect that a tooth has lost its nerve, arrange a dental examination even if the tooth no longer hurts.
What Does It Mean When a Tooth's Nerve Dies?
Inside every healthy tooth is a soft area called the pulp. It contains nerves, blood vessels, and connective tissue.
The pulp can become damaged by:
- Deep tooth decay
- A cracked or fractured tooth
- Severe dental trauma
- Repeated dental procedures
- Significant wear or injury
- Bacterial infection
If the damage becomes severe enough, the pulp can become inflamed and eventually lose its ability to survive.
Once the pulp is no longer viable, the tooth may be described as non-vital or "dead."
Importantly, the hard outer structure of the tooth can remain in place even after the pulp has died.
Does a Dead Tooth Always Hurt?
No.
This is one of the most confusing things about a dead tooth.
A tooth may be extremely painful while its pulp is becoming severely inflamed. But once the pulp tissue loses vitality, the nerve may no longer transmit temperature or pain signals in the same way.
That means a person may think:
"The tooth stopped hurting, so it must be getting better."
Unfortunately, that isn't necessarily true.
If the nerve has died because of an infection or severe damage, bacteria can remain inside the tooth and continue affecting the tissues around its root.
This is why the disappearance of tooth pain does not always mean the underlying problem has resolved.
What Happens After the Nerve Dies?
The process can vary, but several things may happen.
1. The Tooth May Stop Responding to Temperature
A non-vital tooth may no longer respond normally to cold or heat.
A dentist can use specific tests during an examination to determine whether the pulp appears healthy, inflamed, or non-vital.
2. The Tooth May Become Darker
A tooth with damaged or dead pulp can sometimes become gray, yellow, brown, or darker than neighboring teeth.
Discoloration doesn't automatically prove that a nerve has died, because teeth can darken for other reasons. But a sudden color change—particularly after trauma—should be evaluated.
3. Bacteria Can Remain Inside the Tooth
Once the pulp has been damaged or has died, bacteria can colonize the space inside the tooth.
The body's immune system has limited ability to eliminate bacteria from an infected root canal system because the internal space is enclosed within the tooth.
4. Infection Can Develop Around the Root
Bacteria and inflammatory products can move through the root canal system and affect the tissues surrounding the tooth.
This can lead to inflammation around the root and, in some cases, a dental abscess.
5. The Tooth Can Eventually Become Weaker
A tooth that has experienced deep decay, trauma, or extensive structural damage may be more vulnerable to fracture.
A dead tooth itself isn't necessarily brittle simply because its nerve is gone; rather, the extensive decay, cracks, loss of tooth structure, or previous treatment that caused the pulp damage may leave the tooth structurally compromised.
What Are the Warning Signs of a Dead Tooth?
Symptoms vary considerably. Possible signs include:
- Tooth discoloration
- A previous history of severe toothache
- Loss of hot or cold sensitivity
- Pain when biting
- Swelling around the tooth
- A pimple or draining area on the gum
- Persistent bad taste
- Tenderness around the tooth
- No symptoms at all
Because symptoms can be misleading, a dentist may need to perform pulp testing and take dental X-rays to determine what is happening.
Can a Dead Tooth Cause an Abscess?
Yes.
A non-vital tooth can become infected, and infection around the root may lead to a dental abscess.
An abscess can cause:
- Severe or throbbing tooth pain
- Gum swelling
- Facial swelling
- Pus or drainage
- Bad taste
- Pain when chewing
- Fever or feeling unwell
A suspected dental abscess requires prompt professional evaluation.
If facial or neck swelling is rapidly progressing or you have difficulty breathing or swallowing, seek emergency medical attention immediately.
Can a Dead Tooth Be Saved?
Sometimes.
Whether a tooth can be saved depends on factors such as the amount of healthy tooth structure remaining, the extent of infection, cracks or fractures, bone support, and the overall condition of the tooth.
One common treatment for a tooth with infected or irreversibly damaged pulp is root canal treatment.
During root canal treatment, the dentist removes diseased pulp tissue, cleans and disinfects the root canal system, fills the space, and then restores the tooth to help protect it.
In some cases, however, a tooth may be too severely damaged to restore. Extraction may then be recommended.
What Happens If You Don't Treat a Dead Tooth?
A dead tooth shouldn't simply be left alone because it no longer hurts.
If infection is present, it can continue developing even when symptoms are mild or absent. An untreated dental infection may eventually cause an abscess, swelling, bone loss around the tooth, or other complications.
That's why early evaluation is important.
The sooner the underlying cause is identified, the more treatment options may be available.
When Is a Dead Tooth a Dental Emergency?
A non-vital tooth isn't automatically a Dental Emergency, particularly if you have no symptoms. However, certain developments require urgent attention.
Contact an Emergency Dentist if you experience:
- Severe or rapidly worsening tooth pain
- Significant facial or gum swelling
- A dental abscess
- Pus or drainage around the tooth
- Fever associated with dental symptoms
- A severely broken or fractured tooth
- Significant dental trauma
Emergency Dental Service can help patients locate emergency dental providers when urgent dental problems occur.
What Should You Do If You Think Your Tooth Nerve Has Died?
Don't try to diagnose the tooth based only on whether it hurts.
Instead:
Schedule a dental examination. A dentist can determine whether the pulp is still vital and identify the underlying cause.
Avoid chewing hard foods on the affected side if the tooth feels weak or damaged.
Maintain normal oral hygiene. Continue brushing and cleaning between your teeth gently.
Don't place aspirin directly on the tooth or gums. This can irritate or damage oral tissues.
Seek urgent care if swelling or severe pain develops.
When a tooth's nerve dies, the problem isn't necessarily over—it may be moving into a different stage.
The tooth may stop responding to temperature, change color, and eventually develop an infection around the root. Because a non-vital tooth can sometimes cause few or no symptoms, waiting for pain isn't a reliable way to decide whether treatment is needed.
If you notice discoloration, unexplained changes in sensitivity, swelling, or a history of severe tooth pain or trauma, have the tooth examined by a dentist.
And if severe pain, facial swelling, an abscess, or significant dental trauma occurs, seek Emergency Dental Care promptly.
A tooth that stops hurting isn't necessarily a healthy tooth. Sometimes, it's a tooth that needs attention before the next problem begins.
